Please use this format while filling out an unban request. Please note that you will have to make an account on the forums before being able to post a forum thread.
Make sure to have your steam ID when filling out an unban request. This is the only way we can unban you from the server, so it is necessary to have in the unban request.
There are many ways to find your steam ID. Since you made an account on the forums, you can select your account and find your steam ID on the right hand side located under steam details. The other easiest way would be to go to your steam account, copy the steamcommunity link for your account, and go to https://steamidfinder.com/ and paste the link in the search bar. After you click search or press enter, your steam account details will appear.

Reason for Admins to unban you: (If you have proof of your innocence, please attach them or leave a link where we can see them. However, we understand that will most likely not be the case and the staff member(s) should have gathered evidence to justify the ban.)
