I've edited the format in for you. Do you happen to have any screenshots of the logs of the Mass RDM? Or can you include the map name / round this happened on? If you can atleast include those two, I can find it in the damage logs. For future reference when filling these out you should gather screenshots as evidence, as well as follow this template as well.
Just dug up the oldlogs (October/20th/4h/ttt_magma_v2b: Rounds 1-6) and I can see he was RDMing Constantly with No Staff on. This has been handled, thanks for the report!
