Can you grab a screenshot of him leaving? That's all you're missing, otherwise you did your first ban req correctly

He left in between a map change so I dont think it shows in chat

Doesn't really. Maybe try and grab the first round of that map and show that he is not present?

done, next time if they leave during mapvote, check the playerlist by pressing f8 and check if their name is missing on the next map playerlist.
